Thank you for joining us for this special community event! Please review this waiver carefully before participating. Your agreement is required to register and attend.

Assumption of Risk & Waiver of Liability

By participating in this event, I acknowledge that fitness activities, movement classes, and play sessions involve inherent risks — including, but not limited to, physical exertion, falls, contact with equipment, and interactions with other participants.

​

I voluntarily assume all such risks for myself and my child(ren).

​

I understand that it is my responsibility to consult with a qualified healthcare provider before engaging in physical activity — especially if I am pregnant, recently postpartum, or managing any medical condition that could be affected by exercise. I affirm that I have obtained such medical clearance or have chosen to participate at my own risk.

In consideration of being permitted to participate, I hereby release and hold harmless Befriend Body Studio, Playbox Indoor Playground, their owners, instructors, employees, volunteers, and partners from any and all liability, claims, or demands arising out of or related to any injury, illness, property damage, or loss that may occur during or as a result of participation.

Medical & Emergency Acknowledgment

I certify that I and my child(ren) are physically able to participate and have no medical conditions that would prevent safe involvement in fitness or play activities. I authorize emergency medical treatment if deemed necessary.

Participation of Minors

I understand that my child(ren) may participate in play activities within Playbox Indoor Playground or event-related activities. I accept full responsibility for their supervision, safety, and conduct at all times.

​

I acknowledge and agree that this waiver and release applies to my child(ren) as well as to myself, and I hereby release and hold harmless Befriend Body Studio, Playbox Indoor Playground, and their affiliates from any and all liability or claims that may arise from my child(ren)’s participation.

I confirm that my child(ren) are physically able to participate and that I have disclosed any relevant medical or behavioral conditions that may affect their safety or participation.

Media Release

I understand that photos and videos may be taken during the event for use in marketing, social media, or promotional materials by Befriend Body Studio and Playbox Indoor Playground.

I consent to the use of my and/or my child(ren)’s likeness in such materials, provided images are used respectfully and without personal identification.

​

If I prefer that I or my child(ren) not appear in any photos or videos, I will notify event staff upon arrival.

​

Acknowledgment of Agreement

By completing registration and checking the waiver acknowledgment box, I confirm that I have read and agree to the terms above.

​

I understand this digital acknowledgment serves as my electronic signature and that it applies to myself and my participating child(ren).
